Underactive bladder
I have an underactive bladder and was wondering if any studies have been done on using an indwelling catherter for X amount of time to regain muscle tone. I'm also going to ask my urologist about taking a small dosage of Bethanachol (urecholine).
I have notice there is a lot of different views about urecholine on this site. The only problem I can't see for myself is the side affect of low BP. As of now I'm having a hard time voiding then I'm sitting up durning the day. The only way to get myself to void is by pumping my arms up and down. I am a c5 and forehead sweating tells me my bladder is get full enough to start moving my arms, also I just had a spictoromy (sp) 6 weeks which did help some what. Lastly when laying in bed the urine passes much easier, but i still cath in the morning and get between 350 to 450cc, my bladder holds 500. |

Have you been injured a long time? Your bladder is unlikely to do anything but shrink with an indwelling. There is no way to restore tone to a decompensated bladder muscle. Urecholine may cause high pressures (and lots of side effects) and is rarely used any more.
Inability to void when sitting is a common problem in those injured for some time and is generally due to pressure on the urethral below the pubic bone. The only solution is to recline to void. A reclining (not tilt in space) wheelchair may help, as can custom seating. If this does not work, then either routine intermittent cath or an indwelling catheter may be the only solution. (KLD) |

We have had some limited success with a custom molded seat that completely unweights the perineum (area under the pubic bone and behind the scrotum).

recline
luckily i got a reclining chair a month ago, it has tilt and recline, so i tilt back a little and recline with my legs up a bit. i believe this is called zero gravity postion. i don't know why rubbing my chest sets off some decent voiding, 100 or 200cc at a time. i had spinctomy (sp) 6 weeks ago has helped some what i cath once in the morning, this kept me uti free for almost a year, got a uti right after spinctomy, could of been because i stopped catherizing in the morning. voiding after the operation was very good and my bag would fill up quick, now voiding alot at a time has slowed down. i have hadn trouble with my perineum since my injury 27 years ago. http://en.wikipedia.org/wiki/Perineum.
it took about two years for dr. mcquire to figure out that my uretha was getting squashed, so i had to use a sunmate foam cushion with a 4 inch wide by 6 inch long cut out, it was 2 inchs deep. due to a on going pressure sore, got a roho now with the right amount of bubbles tide down in the center. i didn't know when it was purchased that the roho company will customize a cushion by deleting any cell you choose.
