Wise - Spinal cord resource

Could you recommend a book to me that would give a good overview of the anatomy and physiology of the spinal cord and peripheral nervous system?

What I would like to learn would be for example, the neural pathway from the brainstem through the spinal cord, into the peripheral nerve, into the targeted muscle, say the left pointer finger. Also, I would like to learn how this is affected by spinal cord injury. If damage occurred at C-5, and a specific motor neuron died, where would replacement begin, and what would the final target be? Would the axon need only to grow to a neural cell in the peripheral nerve or would it have to grow all the way to the finger? Conversely, what would be the path of repair for an injured sensory pathway if the injury was at C-5?
