Many people have this problem of leaking when they Valsalva or significantly increase their abdominal pressure. This is common in both those with SCI with spastic as well as flaccid bladders. If due to a spastic bladder, anticholenergic medications may help. If it is due to a weak sphincter, procedures such as a bladder sling may be indicated. Urodynamics can be helpful, esp. if done under those same conditions.

If you only have a little dribbling and not a flood, something like a Conveen Drip Collector can be helpful. Many of the wheelchair athletes I work with use these when playing basketball or quad rugby.

I have been leaking like that for the last three years. I wear a pad. sounds crazy but keeps me clean. I leak especially when I work out or place pressure on the stomache just as you said. You are not a lone.
