Where to buy these grade aids?

Does anybody know where I can get a pair of these grade aids/hill holders? I do not want the ones that come with the wheel lock built on. I am going to make them fit a ZR so I will probably have to make a new mount to fit the 1.25" tubing.

Sunrise Medical. I have an extra pair though.
Someone at CC sent me a set and then a new chair came with a set too.
