Help with Tilite part

Could some one help me find out what the part in my Tilite caster is called and how I can order it?

The part I am talking about is well, there is a screw on either side of the caster and then that axle sleeve that they both attach to. Is that what I should call it?

I want to get two new axle sleeves(?) and four replacement screws. I'm not sure what the parts are called or how to ask for them.

I know this may be simple but my brain isn't working very well tonight and I could use the help.

Also, can anyone tell me what bearings I should get to replace mine? I see all sorts of bearings on Ebay but I am not sure what to get for my Tilite TR Softroll wheels.
