burn care? hot grill dropped on foot!
my husband, t6 para, dropped a hot barbecue grill on his bare foot two days ago. there is a blister on one toe, and two lines from the grill....
we've been putting ice on it - any other suggestions? I don't want this blister to turn into a sore! thanks

Burns are not a treat it yourself injury. Ice should not be used at this point. Don't pop blisters but keep them covered with a sterile dressing. If you can get some Silvadene cream, put it on the open areas. Cover with a non-adherant dressing (vasaline gauze, Xeroflow, or Telfa). Change twice daily.
No socks or shoes, no pressure, and keep the sheets off this area. Depending on how deep the burns are, there may be significant tissue sloughing over the next week. Skin grafts are often needed to close a 3rd degree burn. If you see pus in the drainage or there is redness around the burns that increases in area, or if he develops fever, you need to get him to a hospital. Next time be sure he is wearing shoes (leather preferably) when barbecuing. (KLD) |

Thanks for adding that. I should also have mentioned that I have seen some really bad AD from burns for those who are subject to this.

Silvadene is often used, but mainly where there are open wounds. Being notorious for burns, I tend to have this around. Most of the doctors (Physiatrist to Dermatologists) I've seen seem to prefer using OTC antibacterial ointments (eg. Neosporin, Bacitrycin) for healing burns and silvadene for mainly acute/open stage wounds, since silvadene can inhibit healthy cell growth (tho the significance, is debated). Similarly, cleansing open wounds should be done simply with mild soap and water or saline, and not aggressively with any alcohol based products (eg. betadine).
I am prone to AD and can get pretty bad/freq AD with the slightest discomfort, but with burns, my experience is that after the initial contact and body reacts to the pain of the burn (I have no sensation), it doesn't continue to aggravate spasm or trigger out of the ordinary AD. AD and spasms can be good in that it is your body telling you something is wrong, but no need to get too worried about it unless you notice significant changes and are not able to manage it and especially if you become feverish. On the other hand, having little or no AD/spasms do not necessarily indicate there is nothing going on, so one should be careful to be diligent and check skin often to check for PHYSICAL changes in an area (esp. where wounds are observed). Your blisters do not sound too bad. Just do not pop them. Below is pic of one of my less major burns. This has since healed. Some burns may require debridement to remove dead skin. The burn below was not healing and the yellow/greenish layers not sloughing off well, so had to use some debriding ointment (ethezyme). Last edited by chick; 11-05-2006 at 04:13 PM. |

Not much to add other than change the dressing daily, but brings back memories of that cup of coffee I carried in between my legs across the house. Still got scars from that, didnt think that would happen.
