Causes of cord atrophy?

What can cause a consistently atrophic cord, either all cervically or entirely?

Do you mean your spinal cord is not recovering from your injury or that it is progressively getting worse? The doctor who ordered your MRI should be able to give you a better answer as he can compare it to your previous tests.
