    Kidney desease/Kidney failure/Infections/Stones/Pyelonephritis

First of all I regret that I did not read Sinbad post on this forum...
Only couple month ago I went from indwelling(8 years!) to suprapubic.

    http://carecure.org/forum/showthread.php?t=3564

Those who still on indwelling should consider it...

My left kidney is failing and I still do not fully understand why its happen or how it could happen? Last year after I spend 37 days in ICU with very case of phnemonia I've arranged appointment with my urologist Dr.Laplant (here I would say that he is very respected & known among sci community in Montreal, since he works with quads & paras maybe whole his career)...During checkup he noticed that I've developed two stones-one bigger in upper part of kidney & smaller one in lower which was threatening urine flow so he had to put stent.
And he arranged me for surgery in Montreal General in begining of July.I was admitted passed test just to find out that hospital does not have special equipment to retrieve upper stone...So I had to go home & wait three weeks to be admitted to Royal Vic, which had nesessaury tools...In the middle of August I had operation, during which my left lung was punctured-Later resident explained me that surgeon could not access my bigger stone, he had no choice since left kidney is higher than right...So instead spending 1-2 weeks I stayed in hospital more than month...When I was discharged nobody told me about follow ups, but I felt some heavines on my left so I tried to arrange appointment with Dr.Laplant before new year, it was Imposible. I got appointment only in March for routine systoscopy...I told him about heaviness on my left side, he was very concerned, but with his secretary I was scheduled only for the end of the May, thats when I got bad news that Dr. suspecting that my kidney failed.
So I was passing two tests indifferent hospitals(still lack of equipment) Mri scan with Iodine & second something nuclear-very long lest more than 1 hour...

To cut the story short- Dr. Laplant said that his diagnosis Pyelonephrit, my kidney works only 20% of normal capacity, there is no way It going to recover.
Solution is nephrostomy, because Its a "time bomb" could have big infection at any time...

I still don't know what went wrong?
I drink lots of liquids, don't have any spazms or fever, feel myself ok, urine is clear only have depo-like coffee with milk, but not even every morning? Previously I had very low incidence of bladder infections

Some people say-You can live with one kidney....Somehow it does not make me happy

I meet with Dr, again on 4 of July (Independence from Left kidney day

    What should I do?

PS I found interestin article about stats of kidney Desease in North America

"Early Evaluation Critical for Kidney Disease Patients' Survival
    Library: MED
Keywords: KIDNEY DIALYSIS BLACK MALE NEPHROLOGIST
Description: Kidney disease patients are at a much increased risk of death when they have delays getting to a specialist, a Johns Hopkins-led study shows. Delays occur more often among black males, the uninsured and those who have multiple illnesses. (Annals of Internal Medicine, 17-Sep-2002) "

Max, I just wanted to say I'm so sorry to hear of your problems. I don't have any answers for you, but want you to know that you'll be in my thoughts.

Max - Are you saying you should have stayed with the indwelling? Or that you should have switched to the suprapubic earlier?

I'm really concerned how your lung got punctured. Was this a mistake by the surgeon? Sheeeesh.

And I can't believe followups weren't scheduled for you before you left the hospital. I really hope you have quicker turn around times going forward. All the waiting to see specialists obviously works against us. We need regular tests to catch problems early. With one kidney remaining it will be extra critical for you. Don't wait for problems to occur. Each time you have a test reschedule your next one. Maybe you can figure out how to survive within a communist medical system. Don't expect anyone to be looking out for you. They're not. The system is probably overwhelmed so you need to fight for youself.

My best thoughts are with you Bily Maksim.

I hate it when you can't get an appointment for like 3-6 months when you have something serious going on. Its weird you weren't having problems with infections and felt fine up until recently, you should try and find out if it was caused by infections or from the stone causing your kidneys to back up or from pressure over time from your cath not draining properly or whatnot. And isn't a suprapubic catheter just as bad as regular foley indwelling as far as infections, bladder cancer and stone formation, maybe while your having all this surgery you should get them to fix your bladder so you can go to intermitent cathing.

    Max,

I don't have any advice for you, but I just wanted you to know that I am thinking about you and hoping everything goes well for you.

That is terrible that you had to wait so long for an appointment with the specialist. I think when that happens, you have to call them every day and say this is an emergency and you will keep calling until you get in. I know that the ones that make a commotion DO get in quicker because they don't want to keep dealing with you (my friend did that and got bumped up 8 months on the list). You'd think your regular Dr. would have tried to get you in sooner.

Good Luck, Max. I really wish you the best.

    Max,

This must be a most difficult time for you.
Do you think there may be a way to get a second opinion? Perhaps through John Hopkins
    or their equivalent.

Is there anyway to mail/email your urologists
diagnosis/tests to another urologist for consult?

Do they offer lithotripsy (sp) in your country? Could this kidney be functional again if the large upper stone were blasted?
I here that is what lithotripsy does.
Pulverized it into passable dust type particles.

I am so sorry you must suffer and face this.
I guess looking into the past will be good
for the future..but you need innovations now.

My thoughts and prayers are with you.

Karen'sMom - You have a very positive attitude about your health system!

BTW, I continued that vein of discussion in the Life Forum here:

    Karens Mom & Jeff,

You both right but in different ways...

Universal care was first established in Soviet Union after revolution so it could be called "communist" then it was picked up by social democrats in Europe, Even national socialist- Hitler had it...
Mom I don't know how its in BC each province of Canada is different, but its auful here in Quebec, many people say that system started deteriorating approx 1996 when Liberals (federal) made major cuts to healthcare, then Party Quebeqoise (our national-socialists made it worse) I got paralysed in 1995, before never been to hospital ...
Jeff, universal healcare is good thing for common folk if it would be managed properly & funding was adequte...In former SU it was ok...Cuba has very good system (Castro puts funds in it)...
What happen in Quebec is a shame...We have one of the best medical schools in N. America..but many residents I spoke with are planning to leave Quebec for US... Two nurses I knew-one moved to Florida, another to Switzerland....

Nurses in hosptals complain about overload....At the same time former goverment PQ wanted to start building mega -hospital, & new Government Liberals support this idea, despite majority healthcare proffecionals objections...Its just another way to steal funds....Maybe everyone knows story of Montreal Olympic Stadium?

    Lindox,
They got both stones out last year...
I still cannot figure out whats & how its happen, I had no major uties, my blood pressure is always low, no spazms...nada

I'll try to get my files from both hospitals ,but I doubt I could prove any malpractice or negligence...There is very strong corporative solidarity between Drs. here in Quebec...
Thanks...I will definately ask second opinion.....

    Darlene,

I tried "emergency" trick with secretary of my urologist many times...it did not work..Only in march when I had to wait very long in "Live" line, I made a big scandal....

And Thank Everyone for kind attention....
