Is he like a disabled shepard?

That dude isn't in a 2 wheeled wheelchair he's just riding an axle with 2 wheels.

Goat herder. That is pretty rough terrain he wheels through barehanded.
I have had periodic paralysis all my life. I lost my ability to walk in 2011 beginning with a spinal block, which was used for a hip fracture caused by periodic paralysis.
