    OH NO!!! Lost forms from the SSA.. now what?

    Has anyone ever misplaced their SSDI review forms?? I was going to fill them out and send them in today but I looked where I had put it/seen it last and it wasn't there or in any other location that it could possibly be. I called the SSA office from where the form was sent and they told me that I could just wait for the second notice. I am just a little worried that they are just going to think I didn't respond. Has anyone ever needed to wait for a second notice? How long did it take to come in the mail? Was there any repercutions for not turning in the original form?

    Edit: forgot to mention that there is, I think, a 30 day return day window.

    Pick up the phone and call and ask them for a replacement set. Don't wait for them to send a second request.

    There is also the option of going to the SSA office and personally getting the forms you need. Arrive fifteen minutes before the SSA office opens and you should not have a long wait.

    No biggie to call or be there.

    Yes I did call and they said that the forms were not sent out from the location I called. I asked to get the number for that branch. The second person I talked to gave me the number. I called the office in PA and was told that they only send out those forms every so often. It didn't make any sense really but I couldn't think of any other questions to ask her to keep her on the phone. I will call them tomorrow!! Thanks guys. I was/am very nervous about waiting for a second notice. I mean what if there really is no second notice etc.... Thanks again!!

    Really?? I'm shocked slightly since it says on the SSA website that most people are reviewed every three to seven years.

    I saw that....but I cannot remember ever getting reviewed...may ask my ex if she remembers that ever happening.. but I really think I would remember that...

    Me either dj. I got a letter about 5 years after my injury from SSA asking if my doctor would send them something saying I hadn't improved, and wasn't likely to. I called my doc, he sent the letter.

    That was the first and last I've heard about reviews from SSA.
