First - thank you to anyone who can help me better understand what my options are...:o

I had back surgery in May of 2006 - I was 32 years old. I had a herniation at L4-5 addressed. No trouble - until about 6 weeks ago. Same symptoms; extreme sharp pain in lower left hip, left thigh, leg weakness, trembling, numbness. Had a second MRI last week - this one with contrast.

I'm confused.

findings:
...vertebral marrow signal demonstrates discogenic change at the L4-5 level, progressed compared to prior {MRI done 2 years ago}.

Mild residual disc bulging eccentric to the left. There is enhancing scar surrounding the area of disc excision on the left with the findings having led to a moderate mass effect upon the lateral recess, thecal sac, and the left L5 nerve within the lateral recess. Additionally there is question of mild asymmetric enhancement within the left L5 nerve.
At the L5-S1 level there is a hyperintensity within the posterior annulus consistent with annular tear, unchanged from prior.:zombie:

I've read up some on decompression therapy and wonder if it would be a hopeful thing to try. Is surgery an option? Any feedback welcome - thanks again!

You need to get at least two (better 3) opinions from neurosurgeons who specialize in spinal surgery to determine what is your best option. Surgery may be needed if there is neurologic compromise, but there is no guarantee that it will significantly reduce pain.
