It's a trgic story, but it sounds like a case of a manufacturing defect. And the patches are much more the "somewhat effective" for many people who sincerely need that level of medication to at least reduce their pain.

Agreed, it is tragic. There's always a risk involved when dealing with powerful drugs, and quality control is not an easy job, especially when there's tremendous pressure to keep manufacturing costs down. I hope this doesn't result in an increase in the cost of the fentanyl patch to the people who need it - the patch is expensive already, but helps a lot.
