I'm a C5-complete quadriplegic from a car accident w/severe nerve pain off and on over the last year and 10 months. I'm on 1200 mg of gabapentin 4x a day, 175 fentanyl patches every other day, 25mg Nortriptyline 3X a day, and as needed 40mg of Oxycodone 2X and 1mg of Lorazapem as needed, which lately is a lot. I sometimes take more of the gabapentin and the Oxycodone than prescribed to try to get rid of the pain, but it just seems to knock me out.

Please help with any ideas.

Bill, it seems that you are doing the right thing from the viewpoint of pharmaceutical treatments. You are on what 4800 mg of gabapentin which should be in the range where it should help. The fentanyl patches and the oxycodone for breakthrough pain is the next step. The only other treatment that has been reported and that might help is low-dose ketamine and amitryptaline, if you have not tried those before.

I wish that there were more clinical trials of neuropathic pain. I know that there have been a variety of animal studies that have reported other therapies but, not being a pain scientist, I don't know how effective these therapies are. It is one of the most important areas of research that I know and I sincerely hope that it brings forward more and better therapies.

dear bill,
i'm a complete c-6 quad with uncontrollable, constant, excrutiating nerve pain mostly in my butt, back, legs and feet, and somewhat to a lessr degree in myn hands.
i'm on 200 mgs of morphine 2 x day, 900 mgs of gabapenten 4x day, 80 mgs of baclofin a day, 1 mg of clonazapam 3 x day [similar to lorazapam], and at bedtime i take 175mgs of trazodone to help me sleep.
sometimes i break the morphine up to use as breakthrough meds. because my neurologist doesn't believe in breakthrough meds.
my quality of life is nil. i can't sleep longer than 3 houurs at a time, my husband's at his wits end, and is threatening to leave because he says he can't care for me anymore. my fate would be a nursing home.
i don't think he really means it but he's never seemed so serious before.
also, i think my sci doctor has given up on me, he even had the nerve to say he wasn't even sure my pain was real!

well bill, i just wanted you to know i could relaten even though i didn't have any real advice for you except this-
i live in a small suburb 45 miles from the sci doctor in pittsburgh, but i'm STILL looking into chyanging my doc,- you might benefit from getting a second or third opinion.

i don't find many high injury COMPLETEquads like me to talk to,
