Pain: in the body or in the mind? In both!
Summary: While pain manifests both in the body and the mind, therapists only look at the mind side after exhausting physical mechanisms, and patients see too much stigma on "psychologic pain". Pain management by operant conditioning, cognitive-behavior therapy, relaxation techniques, and hypnosis are discussed.
There are very little written on SCI, pain, and psychologic intervention, by Medline I found the Clin J Pain 1992 8:111-8 the best.

Self-hypnosis could be learned from proper books. There are people who can be hypnotized very easy, and others completely resistant to hypnosis (a reason why so many skeptics of hypnosis).
Psychologic methods could be very important complementary therapies in pain management.
