medical neccesity

Yes, This is the deal, I bought my car last friday. I bought a 2001 oldsmobile alero 2dr coupe. I got my handcontrols installed today it looks lovely.. Well I have a dilemma whenever I go to put my car in gear..... I squeeze the button to put the car in drive the button keeps slippin so it take me a couple of minutes just to put the car in gear... So was I thinking about the smartshift to save my time in case of a fast encounter... So do anyone know how I can obtain a medical neccessity form for the smartshift so my insurance can pay for it possibly quick..... NOTE: If someone have a medical neccessity form laying around or on the computer that i can piggy back off..
