Any good large university teaching hospital or trauma center should have a neurosurgeon experienced in management of this. I would check out OSU first, since you are in Ohio, or Cleveland Clinic if you are closer to them.

Keep in mind that surgery for spinal stenosis does carry some significant risks of making paralysis or weakness worse, and this is regardless of the skill of the individual surgeon. You should get at least two opinions before proceeding.

what are chances of paralysis with such a surgery on an able body?

You should ask the surgeons, but I have heard as high as 20% have more impairment after the surgery than before.
