Maybe a lactose or gluten sensitivity? Did you ever do a food and drink log for a week? I know it's a pain in the ass but VERY valuable.
Also, you may want to try some multi-strain probiotics. They will help to try and make your intestines happy.

Hi,
That sound very frustrating- you have been through alot.
I agree with the diet history. If you had the major tests and all is OK (which is really good, as the big bad things have been ruled out), diet may provide the clue.
It also would be best if you could discuss this with a nutritionist. Probiotics are definitely worth a try.
A few questions: are you diabetic? could you have gastroparesis?
Are you using excessive laxatives or meds with your bowel program?

Ditto to all, but also you might look into the quality of the stool samples lab. I was miserable for two years in ways you describe, and it turns out I’d had cryptosporidium. I could trace my symptoms to a day I went hiking with a guy obviously not very adept with his new water filter. Giardia is another quite common parasite. You don’t have to drink the water, just have it splash on something you consume.
Targeted antibiotics and I was reborn.
